I, hereby acknowledge and agree to participate in the Jr. Elite Basketball Summer Camps organized by Unique Hoops Basketball. In consideration for being allowed to participate in the Camp, I agree to the following terms and conditions:
- Assumption of Risk: I understand and acknowledge that participation in basketball activities, including but not limited to drills, games, and other related activities, involves inherent risks and hazards. These risks include, but are not limited to, physical injury, illness, permanent disability, and even death. I voluntarily assume all risks associated with my participation in the Camp.
Release and Waiver: I hereby release, waive, discharge, and hold harmless Unique Hoops Basketball Academy, its owners, directors, employees, agents, contractors, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, arising from my participation in these activities, regardless of whether such loss, damage, or injury is caused by the negligence or fault of Unique Hoops Basketball Academy or any third party. - Indemnification: I agree to indemnify and hold harmless Unique Hoops Basketball Academy from any claims, actions, damages, liabilities, costs, or expenses, including legal fees, arising from or in connection with my participation in the physical recreational activities provided by Unique Hoops Basketball Academy.
- Medical Treatment: In the event of any illness or injury that I may sustain during my participation in the Camp, I authorize the Camp staff to obtain medical treatment on my behalf. I understand that the Camp staff will make reasonable efforts to contact my emergency contact before seeking medical treatment, but in case of an emergency, they may proceed with necessary medical care.
- Insurance: I understand and acknowledge that Unique Hoops Basketball does not provide medical or health insurance coverage for participants. I am solely responsible for obtaining and maintaining my own insurance coverage to cover any expenses related to any injury or illness that I may sustain during the Camp.
- Compliance with Rules and Instructions: I agree to comply with all rules, regulations, and instructions provided by Unique Hoops Basketball and its staff during the Camp. I understand that failure to follow these rules may result in my immediate removal from the Camp without refund.
- Severability: If any provision of this waiver of liability is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.
- Property Damage:I agree that as a parent or legal guardian of the participant, I am solely responsible for any damage done, lost, or stolen property belonging to Unique Hoops Basketball Academy or any other individual caused by their actions during the activities. I understand and accept that Unique Hoops Basketball Academy and its representatives will not be held liable for any such damage, loss, or theft. In the event that any costs are incurred as a result of such damage, loss, or theft, including but not limited to repair or replacement expenses, I agree to assume full financial responsibility. I understand that Unique Hoops Basketball Academy will hold me, as the parent or legal guardian, liable for any and all costs associated with such incidents.
- Governing Law and Jurisdiction: This waiver of liability shall be governed by and construed in accordance with the laws of the [State/Province]. Any legal action arising out of or in connection with this waiver shall be brought in the appropriate courts located within the jurisdiction of [City/County/State/Province].
I have read this waiver of liability and fully understand its contents, and I am of the age 18 years or older. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against Unique Hoops Basketball Academy. I voluntarily and willingly agree to be bound by its terms, and that this cannot be modified orally.

Policies and Procedures for our Jr. Elite Summer Basketball Camps
Registration and Enrollment: I completed the registration process, including providing accurate personal information and emergency contact details.
- Enrollment is on a first-come, first-served basis, and spots are limited.
Code of Conduct:
a. Participants are expected to exhibit respectful and sportsmanlike behavior towards coaches, staff, and fellow participants.
b. Bullying, harassment, or any form of inappropriate behavior will not be tolerated.
c. Participants must follow all camp rules and instructions provided by Unique Hoops staff.
Health and Safety:
a. All participants must provide proof of medical insurance coverage.
b. Participants should inform the camp staff of any pre-existing medical conditions, allergies, or medication requirements.
d. Adequate supervision will be provided during all activities to ensure participant safety.
- Participants should have access to water and rest breaks to prevent dehydration and exhaustion.
ARRIVAL / CHECK-IN / CHECK-OUT:
- Parents MUST bring participants into the facility. All participants will enter and exit through the front main door, the double doors will remain locked.
- Only Authorized individuals on the registration or emergency contact will be required to sign participants in and out of the camp, unless notified by parents/guardian.
- Photo identification may be required for unfamiliar individuals picking up participants.
NO PARENT SIT IN
Parents are not allowed to sit in the camp throughout the course of the camp day.
REFUNDS
After registration, a credit will be used for future programs if there is an injury at the start of your selected program. All credits are good for one year from the date of the canceled session.
COMMUNICATION
All Important information, updates, and schedule changes will be promptly communicated by email parents/guardians.
PRIVACY
Participants' personal information should be kept confidential and used only for camp-related purposes.
CONSENT VIDEO/PHOTOGRAPHY
We have photography and video consent forms in our registration forms and parents must agree before capturing and using any media at Summer Camp.
