Waiver of Liability for Birthday Parties

I hereby acknowledge and agree to participate in the birthday party activities, including moon bounce jumping, organized by Unique Hoops Basketball Academy.  In consideration for being allowed to participate in these activities, I agree to the following terms and conditions:

Assumption of Risk: I understand that participating in physical recreational activities at Unique hoops Birthday parties carries certain inherent risks, including but not limited to, physical injury, property damage, or even death. I voluntarily assume all such risks and acknowledge that Unique Hoops Basketball Academy cannot guarantee my safety during these activities.

Release and Waiver: I hereby release, waive, discharge, and hold harmless Unique Hoops Basketball Academy, its owners, directors, employees, agents, contractors, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, arising from my participation in these activities, regardless of whether such loss, damage, or injury is caused by the negligence or fault of Unique Hoops Basketball Academy or any third party.

Indemnification: I agree to indemnify and hold harmless Unique Hoops Basketball Academy from any claims, actions, damages, liabilities, costs, or expenses, including legal fees, arising from or in connection with my participation in the physical recreational activities provided by Unique Hoops Basketball Academy.

Medical Treatment: In the event of any injury or illness during my participation in these activities, I authorize Unique Hoops Basketball Academy and its staff to administer or seek medical treatment on my behalf, including but not limited to, arranging for emergency medical services, transportation, or hospitalization, if deemed necessary. I understand that I am solely responsible for any medical expenses incurred as a result of such treatment.



Supervision: As a parent or legal guardian of the participant, I understand that it is my responsibility to supervise and ensure the safety of the participant during the activities. I acknowledge that Unique Hoops  is not responsible for providing direct supervision at the birthday party.

Assumption of Risk: I understand and acknowledge that participating in moonbounce jumping and other activities involves inherent risks and hazards. These risks include, but are not limited to, slips, falls, collisions, and other injuries. I voluntarily assume all risks associated with my participation in these activities

I have read this waiver of liability and fully understand its contents. I am aware that by signing this document, I am waiving certain legal rights that I or my heirs, executors, administrators, and assigns may have against the Released Parties. I voluntarily and willingly agree to be bound by its terms.
